Denmark, Novo, and Lilly: The Economics of Weight Loss Drugs

Novo Nordisk was worth more than Denmark's yearly output at the end of 2023, and less than half two years later. In 2025 Lilly's tirzepatide outsold Novo's semaglutide.

Denmark's Ozempic Economy: Novo's Weight in Danish GDP

Denmark's recent growth has come largely from one industry, pharmaceuticals. Statistics Denmark's June 2026 revision puts real GDP growth at 1.1%, 3.9% and 3.5% for 2023 to 2025. Pharmaceuticals supplied 2%, 2.3%, and 1.1% of those rates. In 2023 that exceeded total growth, so the rest of the economy shrank, a split Statistics Denmark calls uncertain. Pharmaceutical gross value added, output minus purchases from other firms, rose from about 6.5% to 8.7% of GDP over those years.

In September 2026, Danmarks Nationalbank attributed strong early-2026 growth to pharmaceutical production abroad. Such growth does not raise gross national income, the income Danish residents earn, by a matching amount.

Novo Nordisk's semaglutide brands include Ozempic, a type 2 diabetes drug, and Wegovy, for weight management. Its 2025 annual report shows a 48% share-price fall and about 9,000 jobs cut, 5,000 in Denmark. Its year-end market value fell from a calculated 113% of Danish GDP in 2023 to 47% in 2025, a comparison of scale only. Denmark's exposure appeared in one company's value and in growth that national income did not match.

How Eli Lilly Overtook Novo Nordisk With Tirzepatide

Lilly sells tirzepatide as Mounjaro, labeled for type 2 diabetes, and Zepbound, labeled for weight management. Lilly's fourth-quarter 2025 results show 2025 sales of $22.97 billion and $13.54 billion, respectively. That $36.5 billion exceeded the roughly $34.5 billion from Novo's three semaglutide brands, converted at Novo's average 2025 exchange rate.

SURMOUNT-5, published by Aronne and colleagues in the New England Journal of Medicine in 2025, suggests one reason. It enrolled 751 adults with obesity but not type 2 diabetes, each on their highest tolerated dose. Over 72 weeks, weight fell 20.2% on tirzepatide and 13.7% on semaglutide. The Lilly-funded trial was open-label, meaning participants knew their drug. Sales data cannot show how much that gap explains, but tirzepatide led in both the trial and 2025 sales.

Novo Nordisk vs Eli Lilly: Two Strategies

Novo built its business on one active ingredient, semaglutide, sold under several brands. Lilly moved faster to a second-generation drug and plans to seek approval for retatrutide, a three-hormone drug, in early 2027. In February 2026 Novo announced that its next candidate, CagriSema, had not proved at least as effective as tirzepatide in REDEFINE 4. Among participants who stayed on treatment in that 84-week open-label trial, weight fell 23.0% on CagriSema and 25.5% on tirzepatide.

On pills, Novo moved earlier. Its 2025 annual report records US approval of the Wegovy pill, while Lilly's second-quarter 2026 results record first sales of its pill, Foundayo. Announced US manufacturing plans also differ. The White House Most-Favored-Nation Pricing Fact Sheet of November 2025 lists at least $27 billion from Lilly and an additional $10 billion from Novo. Neither figure measures capacity.

Recent results favor Lilly. Novo's first-half 2026 interim report forecasts 2026 adjusted sales between flat and 6% lower, excluding currency effects. Lilly's second-quarter 2026 revenue rose 48% year on year. Yet Lilly's Form 10-K puts Mounjaro and Zepbound at 56% of its 2025 revenue. Lilly now depends on one active ingredient as Novo did.

Why Zepbound and Mounjaro Cost So Much

Lilly can charge more than $1,000 for a 28-day supply because a patent protects tirzepatide. Lilly estimates its US patent runs to 2036, and no generic can be sold before then. Its Form 10-K says it bills wholesalers at list price, then pays rebates and discounts, so list price overstates its receipts.

Lilly's Zepbound and Mounjaro pricing pages, checked October 29, 2026, list Zepbound at $499 to $1,086.37 per 28-day fill and Mounjaro at $1,112.16. LillyDirect's self-pay cash prices for Zepbound are $299 for the 2.5-mg starting dose and $399 for 5 mg. Doses of 7.5 to 15 mg cost $449 with refills within 45 days under the Zepbound Journey Program, otherwise $499 or $699. The $449 price is about 41% of Zepbound's top list price.

Paying for weight loss invites the objection that it is bought rather than earned. Whether taking Mounjaro or Ozempic is cheating is examined in a separate article.

Compounded Tirzepatide

Compounded tirzepatide is made by a pharmacy without approval from the US Food and Drug Administration (FDA). It usually costs less, roughly at $129 to $439 a month, with a median of $299. Zepbound's lowest maintenance self-pay price is $449.

FDA's page clarifying its policies for compounders says the tirzepatide shortage ended on December 19, 2024. Copies of an approved drug are now restricted to a prescriber-documented significant difference for an identified patient, or four or fewer prescriptions a month.

FDA's page on unapproved glucagon-like peptide-1 (GLP-1) drugs says the agency "does not review compounded drugs for safety, effectiveness or quality." It counts more than 730 adverse-event reports for compounded tirzepatide by May 31, 2026. It tells patients to get a licensed doctor's screening and prescription, use a state-licensed pharmacy and refuse injectables that arrive warm. Among tirzepatide treatment options , Mounjaro and Zepbound are FDA-approved and compounded tirzepatide is not.

Zepbound's list and self-pay prices are two prices for one drug from a company that depends on it.
